Important Information |

The following students CANNOT apply for transcript under CUSIS at the moment due to system limitation. They should apply by e-mail to This email address is being protected from spambots. You need JavaScript enabled to view it. |

Students / graduates of the following programmes should submit their applications to the respective offices: |

Application for e-Copy Transcript |

• |
Except those students listed under point 1, all applications MUST be submitted under CUSIS using the function “e-Copy Transcript Application” after login to MyCUHK (https://portal.cuhk.edu.hk/). |

Graduates need to use Alumni ID and MyCUHK password to login MyCUHK. Graduates may refer to the homepage of the Alumni Affairs Office (AAO) (https://www.alumni.cuhk.edu.hk/MyCUHK/RetreiveAlumniID_ResetPassword.html) for details. Graduates may also click this link to watch the video of AAO for quick reference. |

e-Copy Transcript is to be sent by e-mail to the institutions, prospective employers or students themselves. Please indicate CLEARLY the email address, the name of the recipient and the purpose of the application. Applicants must check beforehand whether the recipient accepts e-Copy transcript as an email attachement. If the applicants need to include specified form from the university / institution to be completed by the University and sent together with the transcript, they MUST upload the related document(s) during submission of the application at the same time. |

Transcripts in Chinese version (only applicable to applicants who graduated in 2011 and thereafter) are available. Applicants MUST notify our Section by email (This email address is being protected from spambots. You need JavaScript enabled to view it.) within the same day of submitting the application of this special requirement. Longer processing time is required. |

No hardcopy version of the transcript will be provided if applicants apply for e-Copy transcript. If hardcopy version is needed, applicants should apply for hardcopy transcript via the function “Hardcopy Transcript Application” under CUSIS. |

Current students may consider downloading the unofficial transcript under CUSIS for personal use / retention. |

i. |
Once the applications have been completed, applicants are not allowed to make changes to or cancel their applications (see point 4 below). They should submit a new application if there are any changes to their applications. |

Our Section accepts no responsibility for any loss or delay in delivery of the documents due to unforeseeable / technical problems. |

Fee Schedule |

|
Fee per single copy is HK$50. |

Processing Time |
|||||||||||||||
|
• |
Normal processing time is 3-5 working days. Some applications (e.g., student records of the applicants were not yet computerized during their period of study) may take longer processing time. |

|
• |
Please enquire application progress using the function “View Application Status” of "e-Copy Transcript Application" under MyCUHK. “Completed” will be marked when the processing has been finished. |

May I request a letter certifying that the medium of instruction at the University is English?
As CUHK is a bilingual university, the letter of certification will certify, in general, that courses were taught in English and/or Chinese (except cases on the contrary).

What should I do if I have to include documents to be sent together with my transcript / I need to put the transcript in sealed envelope?
If the applicants need to include the specified form from the university / institution to be completed by our University and sent together with the transcript, they MUST upload the related document(s) when submitting their applications. For requests of hardcopy transcript to be inserted in sealed envelope and returned back to the applicants, applicants should inform our Section by email before submitting the application.

May I request a copy of document to be sent by e-mail?
Applicants may request sending the completed PDF document by e-mail. Please indicate CLEARLY the email address, the name of the recipient and the purpose of the application. Applicants must check beforehand whether the recipient accepts the document as an email attachment.
Current students may consider downloading the unofficial transcript under CUSIS for personal use / retention.

Can I cancel my application after submission?
Once the applications have been completed, applicants are not allowed to make changes to or cancel their applications. They should submit a new application if there are any changes to their applications. Please enquire application progress via “View Application Status” of the relevant functions under MyCUHK. The system will mark “Completed” if processing has been finished.

May I request collection of hardcopy documents in person?
Please notify our Section of this request by email (This email address is being protected from spambots. You need JavaScript enabled to view it.) within the same day of submitting the application so that our Section can make the relevant arrangement.

Application for Certified True Copy of Degree Certificate |

Important Information |

• |
Graduates of undergraduate programmes (other than those of MBChB and non-credit-unit-based Nursing programmes) who have collected the degree certificates may request for a certified true copy by bringing along the original certificates in person or by an authorized person to the Registration and Examinations Section for certification. |
||||||||||||
|
• |
Graduates of MBChB and non-credit-unit-based Nursing programmes should enquire at the Faculty and Planning Office, Faculty of Medicine. |

Application for Certified True Copy of Degree Certificate |

• |
The original certificate MUST be presented in person or by an authorized person for certification. |

• |
All applications MUST be submitted under CUSIS using the function “e-Copy Certified True Copy of Degree Certificate Application” or “Hardcopy Certified True Copy of Degree Certificate Application” as appropriate under “UG Applications” of “MyPage” after login MyCUHK (https://portal.cuhk.edu.hk/). Please refer to System Walkthrough for details. |

Graduates of current academic term should only submit applications for the certified true copy AFTER the degrees have been conferred to them AND they have collected their degree certificates. |

No hardcopy version of the certified true copy will be provided if applicants apply for e-Copy. If hardcopy version is needed, applicants should apply for hardcopy certified true copy via the function “Hardcopy Certified True Copy of Degree Certificate Application” under CUSIS. |

Fee per single copy is HK$50. |

For Hardcopy |
||||||||||||
Fee per single copy, including postage of local or ordinary air mail is HK$50 (e.g. if you apply for 2 copies of the document, the payment shall be HK$100). Applicants will need to pay an additional charge as follows for sending hardcopy certified true copy of degree certificate using other postal services:

Processing Time |
|||||||||||||

• |
Normal processing time is 2-3 hours upon presentation of the original degree certificate and receipt of the application. Applicants are strongly advised to come to our counter in the morning or in the early afternoon. The original certificate will be returned to the applicant on the same day. For e-Copy, the certified true copy will be issued by email by the end of the next working day. For hardcopy, the certified true copy will be passed to the applicant together with the original certificate. If mailing service is requested for the certified true copy, an additional 1-2 working days will be required for further processing. |

Applicants may enquire application progress using the function “View Application Status” of “e-Copy Certified True Copy of Degree Certificate Application” / “Hardcopy Certified True Copy of Degree Certificate Application” under MyCUHK. “Completed” will be marked when the application is finished. For applications for hardcopy, applicants may also enquire the tracking number in the applications for checking the mailing progress on Hongkong Post website if the hardcopy documents are sent by registered mail or Speedpost. |
