Application for Certified True Copy of Degree Certificate |

1. |
Important Information |
|||||||||||||
|
• |
Graduates of undergraduate programmes (other than those of MBChB and non-credit-unit-based Nursing programmes) who have collected the degree certificates may request for a certified true copy by bringing along the original certificates in person or by an authorized person to the Registration and Examinations Section for certification. |
||||||||||||
|
• |
Graduates of MBChB and non-credit-unit-based Nursing programmes should enquire at the Faculty and Planning Office, Faculty of Medicine. |

2. |
Application for Certified True Copy of Degree Certificate |

• |
The original certificate MUST be presented in person or by an authorized person for certification. |
||||||||||||
|
• |
All applications MUST be submitted under CUSIS using the function “e-Copy Certified True Copy of Degree Certificate Application” or “Hardcopy Certified True Copy of Degree Certificate Application” as appropriate under “UG Applications” of “MyPage” after login MyCUHK (https://portal.cuhk.edu.hk/). Please refer to System Walkthrough for details. |

Graduates need to use Alumni ID and MyCUHK password to login MyCUHK. Graduates may refer to the homepage of the Alumni Affairs Office (AAO) (https://www.alumni.cuhk.edu.hk/MyCUHK/RetreiveAlumniID_ResetPassword.html) for details. |

Graduates of current academic term should only submit applications for the certified true copy AFTER the degrees have been conferred to them AND they have collected their degree certificates. |

• |
No hardcopy version of the certified true copy will be provided if applicants apply for e-Copy. If hardcopy version is needed, applicants should apply for hardcopy certified true copy via the function “Hardcopy Certified True Copy of Degree Certificate Application” under CUSIS. |

i. |
Once the applications have been completed, applicants are not allowed to change or cancel their applications (see point 4 below). They should submit a new application if there are any changes in their applications. |

Our Section accepts no responsibility for any loss or delay in delivery of the documents due to unforeseeable / technical problems. |

Fee Schedule |

For e-Copy |
||||||||||||
Fee per single copy is HK$50. |
||||||||||||||
|
ii. |
For Hardcopy |
||||||||||||
Fee per single copy, including postage of local or ordinary air mail is HK$50 (e.g. if you apply for 2 copies of the document, the payment shall be HK$100). Applicants will need to pay an additional charge as follows for sending hardcopy certified true copy of degree certificate using other postal services:

Processing Time |
|||||||||||||

• |
Normal processing time is 2-3 hours upon presentation of the original degree certificate and receipt of the application. Applicants are strongly advised to come to our counter in the morning or in the early afternoon. The original certificate will be returned to the applicant on the same day. For e-Copy, the certified true copy will be issued by email by the end of the next working day. For hardcopy, the certified true copy will be passed to the applicant together with the original certificate. If mailing service is requested for the certified true copy, an additional 1-2 working days will be required for further processing. |

Applicants may enquire application progress using the function “View Application Status” of “e-Copy Certified True Copy of Degree Certificate Application” / “Hardcopy Certified True Copy of Degree Certificate Application” under MyCUHK. “Completed” will be marked when the application is finished. For applications for hardcopy, applicants may also enquire the tracking number in the applications for checking the mailing progress on Hongkong Post website if the hardcopy documents are sent by registered mail or Speedpost. |
