Re: Important: Verification of Personal Information for Graduation Records
Dear Graduating Student,
As you are expected to graduate in 2nd term 2022-23, please check your registered name BOTH in Chinese and English under CUSIS (click Profile > Personal Details). This is to make sure that your name in English and Chinese (if applicable) will be correctly printed on your graduation transcript, completion letter and degree certificate.
Please note that, in accordance with University Regulation (Clause 1.4), a student shall register in the name which appears in his/her Hong Kong Identity Card or passport. No application for changing a student’s registered particulars shall be accepted after s/he has been recommended by the Undergraduate Examinations Board for graduation, unless under very special circumstances. Therefore, if you need to change your registered name under CUSIS, please submit in person on or before 7 July 2023 the application form with documentary proof to the Registration and Examinations Section (for MBChB students, to the Faculty and Planning Office of the Faculty of Medicine). Please also check carefully your HOME address record under CUSIS and amend it if necessary by 7 July 2023, so that the graduation transcript and completion letter to be sent to you after the release of degree results will not be lost in mail.
Please be reminded once again that the personal data captured during your undergraduate studies (e.g. name in English and Chinese, home address record, etc.) will be converted as alumni data for the use of the Alumni Affairs Office of the University in accordance with appropriate guidelines, after the approval of the University Senate for graduation records.
