Application for Report on Curriculum Details
(for Undergraduate Programmes)

1. Important Information

  • The following students CANNOT apply for report on curriculum details under CUSIS at the moment due to system limitation. They should apply by e-mail to transcript@cuhk.edu.hk

– Associate students

– Students who withdrew from the University and did NOT complete the degree programmes

  • Students / graduates of the following programmes should enquire at the respective offices:

– MBChB programme and non-credit-unit-based Nursing graduates: Faculty and Planning Office, Faculty of Medicine

– International Asian Studies Programme (IASP): Office of Academic Links

– (Advanced) Postgraduate Diploma in Education programme: Faculty Office of Education

– Other postgraduate programmes: Graduate School Office

2. Report on Curriculum Details

  • A report on curriculum details shows the course descriptions, if available, in the Student Handbook of the courses taken by the students during their years of attendance.

3. Application for Report on Curriculum Details

  • Except those students listed in point 1, all applications MUST be submitted under CUSIS using the function “e-Copy Report on Curriculum Details Application” or “Hardcopy Report on Curriculum Details Application” as appropriate after login MyCUHK (https://portal.cuhk.edu.hk).

i. For current students: under “Applications” of CUSIS

ii. For graduates: under “UG Applications” of “MyPage”

Please refer to System Walkthrough for details.

  • Graduates need to use Alumni ID and MyCUHK password to login MyCUHK.  Graduates may refer to the homepage of the Alumni Affairs Office (AAO) (https://www.alumni.cuhk.edu.hk/MyCUHK/RetreiveAlumniID_ResetPassword.html) for details.
  • No hardcopy version of the report on curriculum details will be provided if applicants apply for e-Copy.  If hardcopy version is needed, applicants should apply for hardcopy report on curriculum details via the function “Hardcopy Report on Curriculum Details Application” under CUSIS.

Note:

i. Once the applications have been completed, applicants are not allowed to change or cancel their applications (see point 5 below). They should submit a new application if there are any changes in their applications.

ii. Our Section accepts no responsibility for any loss or delay in delivery of the documents due to unforeseeable / technical problems.

4. Fee Schedule

i. For e-Copy Report

Fee for the first copy per application is HK$150, and HK$50 for each subsequent copy in the same application (e.g. if you apply in the same application for 2 copies of report on curriculum details, the payment shall be HK$200).

ii. For Hardcopy Report

Fee for the first copy per application is HK$150, including postage of local or ordinary air mail, and HK$50 for each subsequent copy in the same application (e.g. if you apply in the same application for 2 copies of report on curriculum details, the payment shall be HK$200).

Applicants will need to pay an additional charge as follows for sending hardcopy report on curriculum details using other postal services:

5. Processing Time

  • Normal processing time is 15 working days.
  • Applicants may enquire application progress using the function of “View Application Status” of “e-Copy Report on Curriculum Details Application” / “Hardcopy Report on Curriculum Details Application” under MyCUHK. “Completed” will be marked when the application is finished. For applications for hardcopy report, applicants may also enquire the tracking number in the applications for checking the mailing progress on Hongkong Post website if the hardcopy documents are sent by registered mail or Speedpost.
