Dear Students,
As you are expected to complete your studies in the 2nd term 2025-26, please be reminded to check your registered name in BOTH English and Chinese under CUSIS (click Profile > Personal Details) to make sure that your name in English and Chinese (if applicable, and please pay special attention to the variant of Chinese characters, that the ones as appeared in CUSIS should be the same as in your HKID card) can be correctly printed on your academic transcript, letter of completion and degree certificate.
Please note that Regulation 1.4 of the General Regulations Governing Full-time Undergraduate Studies stipulates: “A student shall register in the name which appears in his/her Hong Kong Identity Card or passport. Only under very special circumstances may an applicant who has been recommended by the Undergraduate Examinations Board for graduation or has withdrawn from the University apply for change of his/her registered particulars. Such an application shall be made in writing with justification for approval by the Registrar.” If you need to make changes to your name as registered in the University’s record before graduation, please submit the application in person using this application form with documentary proof to the Registration and Examinations Section (for MBChB students, to the Faculty and Planning Office of the Faculty of Medicine) on or before 12 June 2026. In case you notice that the Chinese character(s) variant recorded in CUSIS differ(s) from what is shown on your HKID card, please also notify us by email (with a copy of HKID card) / phone by the same date. Any applications for change of registered particulars will NOT be accepted after the specified deadline, unless there are very special justifications.
Please also check carefully your HOME address recorded under CUSIS and update it as necessary by 12 June 2026. Kindly note that a copy of the academic transcript and letter of completion will be sent by mail to your HOME address after you have been approved for graduation in July 2026.
